Monthly Contract Policy Service Overview Steven’s Transport & Care Services LLC provides professional monthly contract sevice. Services are completed in a timely, confidential, and professional manner. Scheduling All pickups and deliveries must be scheduled in advance. Same day and urgent requests are subject to availability and may result in additional fees. Clients must provide accurate pickup and delivery information at the time of scheduling. Authorized Items: Services may include: Routine prescription medications Same-day prescription deliveries Refill pickups PPE (gloves, masks, protective gear) Wound care supplies (bandages, gauze, dressings) Incontinence supplies (briefs, pads) Compounded medications Specialty medications Antibiotics and short-term Diabetic testing supplies (meters, test kits, strips) Mobility aids (walkers, canes, crutches) Wheelchairs, Oxygen machines & more The company reserves the right to refuse transportation of prohibited, unsafe, or unauthorized items. Pickup & Delivery Requirements Medications and supplies must be properly packaged and ready for transport at the scheduled pickup time. Clients are responsible for ensuring prescriptions are available for release by the pharmacy or provider. Signature confirmation may be required upon delivery when applicable. Wait Time & Delays Delays caused by pharmacies, providers, weather, traffic, or unforeseen circumstances may affect delivery times. Cancellations Cancellations should be made 12 hours prior to start time. Payment Payment is due prior to or at the time of service unless a contract or billing agreement is in place. Monthly plans and contract services are non-transferable and must be used within the agreed service period. Liability Steven’s Transport & Care Services LLC is responsible for transporting approved items safely and professionally but is not responsible for: Incorrect prescriptions provided by the pharmacy Issues outside the company’s control after delivery confirmation
